What is New Skin Needle (Saesalchim)?

Deep within the skin, a pulling force acts on the scar, drawing it downwards.

A seemingly depressed scar actually has

a fibrous tether that strongly pulls the scar downwards from beneath the dermis.

Only by releasing this adhesive tissue can the depressed skin return to its original position.

The procedure that incises and releases these adhesive structures is Saesalchim, also known as subcision.

How Saesalchim Works

1. Release of Adhesions

The fibrous tethers located deep within the skin

are carefully incised using a special needle or cannula.

→ This releases the skin that was pulled down, allowing the depressed scar area to refill.

2. Micro-Circulation Stimulation (Regenerative Signal)

The subtle bleeding induced during the release of adhesions

acts as a key stimulus to promote new collagen synthesis.

→ Through this stimulation, fibroblasts are activated, laying the groundwork for new tissue generation in the scar area.

3. Improved Blood Circulation and Tissue Regeneration Promotion

In some cases, combining with microneedling or PDRN (DNA injection) treatment

can further activate blood circulation and the influx of regenerative cells to the affected area,

thereby aiding both scar recovery and new collagen formation simultaneously.

What types of scars can be considered for Saesalchim treatment?

Deep, pitted scars on the forehead, resembling dots

Scars on the temples that appear deeply indented and uneven

What is Multi Saesalchim? (Adding Erbium YAG Shouldering to existing Saesalchim)

This procedure combines traditional subcision (Saesalchim) with the shouldering technique.

During scar treatment, simply releasing the adhesive tissue beneath the scar may not be sufficient.

Even after releasing adhesions with subcision, scars with distinct and deep borders may face difficulties in achieving smooth skin surface regeneration.

Our bodies have a natural tendency to match the height and texture of surrounding skin when healing damaged tissue.

However, if the scar borders drop abruptly, this natural healing mechanism may not function properly.

For this reason, we delicately refine the scar borders to create a gentle slope, which is called shouldering.

By reducing the height difference with surrounding skin through shouldering, when the scar refills, it can blend more naturally with the surrounding skin, leading to a less noticeable result.

Indeed, cases where sufficient improvement was not achieved with traditional Saesalchim alone are closely related to this.

This is because even if the underlying adhesions are released, if the borders remain like a wall, the skin's natural regeneration process will be restricted. 

Therefore, shouldering can be considered an important step in treating clearly defined depressed scars, complementing the limitations of existing procedures. 

Cases where Multi Saesalchim can be beneficial

1) Those who have not experienced sufficient improvement with traditional Saesalchim or general scar treatments alone

There are those who are concerned that the scar area has not improved as much as expected even after existing subcision or laser treatments.

This is because even if the bottom part of the scar has been released, if the edges still remain steep, it may be difficult for the skin to regenerate on its own.

In this case, by gently softening the scar boundaries with a needling shoulder treatment, more natural improvement can be expected.

2) If the scar is deeply indented with clear and steep boundaries 

Even if the skin regenerates from within, forms with sharply indented boundaries, such as boxcar scars (sharply depressed scars),

may not allow for smooth overall recovery due to the remaining wall-like boundaries on the sides.

Erbium YAG needling shoulder treatment softens the steep edges of scars, thereby reducing the difference in height with surrounding skin and helping to achieve a more natural skin texture.

3) If you expect skin improvement with fewer sessions than traditional scar regeneration treatments

Typical scar regeneration treatments require multiple repeated sessions to feel gradual improvement. However, multi-pronged scar regeneration treatment is performed by comprehensively considering the adhesion site, scar boundaries, and surface.

Therefore, it is expected that you can experience improvement with relatively fewer sessions compared to typical scar regeneration treatments.
